Partnership Walk provides a signed Release of Liability and Consent Form that must be reviewed and accepted. Please read the following Release of Liability and Consent Form before participating in any Partnership Walk activities. RELEASE OF LIABILITY & CONSENT FORM In consideration of my participation in Partnership Walk to be sponsored by Aga Khan Foundation U.S.A. I, the undersigned, intending to be legally bound, hereby, for myself, my heirs, executors and administrators, release, waive and discharge Aga Khan Foundation U.S.A., its bonafide members and any of the organizers, sponsors, and officials of the said Event, and any of their respective representatives, agents, successors and assigns, for any and all injuries, damages, or death suffered by me in connection with my participation in the Event. I hereby assume full responsibility for all risk of injury, damage or death in connection with the said Event. I hereby consent to any medical or surgical treatment which I may need and which arises in connection with the said Event; provided, however, nothing contained herein shall be deemed an obligation to provide any such medical or surgical treatment. This site is intended for the lawful use of Aga Khan Foundation U.S.A.’s supporters, employees and members of the general public who are over the age of 18.
